Therefore, we’ve created a wellbeing referral programme that looks at your overall health and help you get into exercise and find and will create you an activity programme that will benefit your overall health and wellbeing.
Wellbeing referral scheme
Our wellbeing referral scheme aims to provide opportunities for people with underlying health conditions, those who are inactive or those who have been identified to be higher risk of developing conditions to become more active. Our centres are a safe, friendly and are the perfect place to start your journey and help you create long term behaviour change that will positively affect all aspects of your life.
This programme will help people lead a healthier, happier life through a variety of physical and social activities.
How to join the wellbeing referral scheme?
There are two pathways to join our Wellbeing referral scheme. You can be referred by one of our partners listed below, who will send us your information, we’ll then contact you to get started on our referral programme.

- Essex County Council – Health & Wellbeing Services
- Approved Local GP surgeries
- Approved Local Physiotherapists
Alternatively, you can self refer onto our scheme if you have one of the following health conditions.
- Diabetes or pre-diabetes
- High Cholesterol
- Coronary Heart Disease
- High Blood Pressure
- Long Covid
- Lung Disease
- Osteopenia or Osteoporosis
- Osteo/Rheumatoid Arthritis
- Joint or Musculoskeletal disorder
- Mental Health
What extra help will we give you?
You will be supported over the next 12 months once you start with us where we will support you through your health and fitness journey.
Once we’ve contacted you to get started, you’ll be invited for an informal consultation with one of our expert team members. During this time, we’ll chat about your health & fitness goals, health and exercise history, your likes and dislikes and make you feel at ease in this new environment.
You’ll leave with an activity plan and advice on areas that will improve your mental and physical health. This isn’t where we leave you though.
Throughout the next 12 months you will be able to attend sessions with people in the same position as you and book in for support with members of the team for motivation, advice and updates to your activity plan as you progress.
All of this will result in you including exercise into your weekly routine and forming a habit with our help and you’ll never look back. You’ll get fitter, be more social, feel happier and live better with our help.
